Make sure all non-READ SCSI commands get targ_xfer_tag initialized
to 0xffffffff, not just WRITEs.
Double-free of a TUR cmd object occurs under the following scenario:
1. TUR received (targ_xfer_tag is uninitialized and left at 0)
2. TUR status sent
3. First unsolicited NOPIN is sent to initiator (gets targ_xfer_tag of 0)
4. NOPOUT for NOPIN (with TTT=0) arrives
- its ExpStatSN acks TUR status, TUR is queued for removal
- LIO tries to find NOPIN with TTT=0, but finds the same TUR instead,
TUR is queued for removal for the 2nd time

This patch fixes a use-after-free bug in iscsit_release_sessions_for_tpg()
where se_portal_group->session_lock was incorrectly released/re-acquired
while walking the active se_portal_group->tpg_sess_list.
The can result in a NULL pointer dereference when iscsit_close_session()
shutdown happens in the normal path asynchronously to this code, causing
a bogus dereference of an already freed list entry to occur.
To address this bug, walk the session list checking for the same state
as before, but move entries to a local list to avoid dropping the lock
while walking the active list.
As before, signal using iscsi_session->session_restatement=1 for those
list entries to be released locally by iscsit_free_session() code.

pscsi_configure_device(). In VIRTUAL_HOST mode it is associated with

This patch changes iscsit_do_tx_data() to fail on short writes
when kernel_sendmsg() returns a value different than requested
transfer length, returning -EPIPE and thus causing a connection
reset to occur.
This avoids a potential bug in the original code where a short
write would result in kernel_sendmsg() being called again with
the original iovec base + length.
In practice this has not been an issue because iscsit_do_tx_data()
is only used for transferring 48 byte headers + 4 byte digests,
along with seldom used control payloads from NOPIN + TEXT_RSP +
REJECT with less than 32k of data.
So following Al's audit of iovec consumers, go ahead and fail
the connection on short writes for now, and remove the bogus
logic ahead of his proper upstream fix.

During temporary resource starvation at lower transport layer, command
is placed on queue full retry path, which expose this problem. The TCM
queue full handling of SCF_TRANSPORT_TASK_SENSE currently sends the same
cmd twice to lower layer. The 1st time led to cmd normal free path.
The 2nd time cause Null pointer access.
This regression bug was originally introduced v3.1-rc code in the
following commit:

In iscsi_copy_param_list() a failed iscsi_param_list memory allocation
currently invokes iscsi_release_param_list() to cleanup, and will promptly
trigger a NULL pointer dereference.
Instead, go ahead and return for the first iscsi_copy_param_list()
failure case.

a pointer used as storage for list_for_each_entry() was incorrectly
being used to determine if no matching entry had been found.
This patch changes iscsit_logout_post_handler_diffcid() to key off

This patch changes rd_allocate_sgl_table() to explicitly clear
ramdisk_mcp backend memory pages by passing __GFP_ZERO into
alloc_pages().
This addresses a potential security issue where reading from a
ramdisk_mcp could return sensitive information, and follows what
>= v3.15 does to explicitly clear ramdisk_mcp memory at backend
device initialization time.

This patch adds an explicit check in chap_server_compute_md5() to ensure
the CHAP_C value received from the initiator during mutual authentication
does not match the original CHAP_C provided by the target.
This is in line with RFC-3720, section 8.2.1:
Originators MUST NOT reuse the CHAP challenge sent by the Responder
for the other direction of a bidirectional authentication.
Responders MUST check for this condition and close the iSCSI TCP
connection if it occurs.

This patch changes vectored file I/O to use kmap + kunmap when mapping
incoming SGL memory -> struct iovec in order to properly support 32-bit
highmem configurations. This is because an extra bounce buffer may be
required when processing scatterlist pages allocated with GFP_KERNEL.

This patch fixes the MAINTENANCE_IN service action type checks to only
look at the proper lower 5 bits of cdb byte 1. This addresses the case
where MI_REPORT_TARGET_PGS w/ extended header using the upper three bits of
cdb byte 1 was not processed correctly in transport_generic_cmd_sequencer,
as well as the three cases for standby, unavailable, and transition ALUA
primary access state checks.
Also add MAINTENANCE_IN to the excluded list in transport_generic_prepare_cdb()
to prevent the PARAMETER DATA FORMAT bits from being cleared.

There are some cases, for example when the initiator sends an
out-of-bounds ErrorRecoveryLevel value, where the iSCSI target
terminates the connection without sending back any error. Audit the
login path and add appropriate iscsit_tx_login_rsp() calls to make
sure this doesn't happen.

ft_del_tpg checks tpg->tport is set before unlinking the tpg from the
tport when the tpg is being removed. Set this pointer in ft_tport_create,
or the unlinking won't happen in ft_del_tpg and tport->tpg will reference
a deleted object.
This patch sets tpg->tport in ft_tport_create, because that's what
ft_del_tpg checks, and is the only way to get back to the tport to
clear tport->tpg.
The bug was occuring when:
- lport created, tport (our per-lport, per-provider context) is
allocated.
tport->tpg = NULL
- tpg created
- a PRLI is received. ft_tport_create is called, tpg is found and
tport->tpg is set
- tpg removed. ft_tpg is freed in ft_del_tpg. Since tpg->tport was not
set, tport->tpg is not cleared and points at freed memory
- Future calls to ft_tport_create return tport via first conditional,
instead of searching for new tpg by calling ft_lport_find_tpg.
tport->tpg is still invalid, and will access freed memory.

This patch fixes a long-standing bug in iscsit_build_conn_drop_async_message()
where during ERL=2 connection recovery, a bogus conn_p pointer could
end up being used to send the ISCSI_OP_ASYNC_EVENT + DROPPING_CONNECTION
notifying the initiator that cmd->logout_cid has failed.
The bug was manifesting itself as an OOPs in iscsit_allocate_cmd() with
a bogus conn_p pointer in iscsit_build_conn_drop_async_message().

This patch re-adds the ability to optionally run in buffered FILEIO mode
(eg: w/o O_DSYNC) for device backends in order to once again use the
Linux buffered cache as a write-back storage mechanism.

Convert to use O_DSYNC for all cases at FILEIO backend creation time to
avoid the extra syncing of pure timestamp updates with legacy O_SYNC during
default operation as recommended by hch. Continue to do this independently of
Write Cache Enable (WCE) bit, as WCE=0 is currently the default for all backend
devices and enabled by user on per device basis via attrib/emulate_write_cache.
This patch drops the now unnecessary fd_buffered_io= token usage that was
originally signalling when to explictly disable O_SYNC at backend creation
time for buffered I/O operation. This can end up being dangerous for a number
of reasons during physical node failure, so go ahead and drop this option
for now when O_DSYNC is used as the default.
Also allow explict FUA WRITEs -> vfs_fsync_range() call to function in
fd_execute_cmd() independently of WCE bit setting.

When creating network portals rapidly, such as when restoring a
configuration, LIO's code to reuse existing portals can return a false
negative if the thread hasn't run yet and set np_thread_state to
ISCSI_NP_THREAD_ACTIVE. This causes an error in the network stack
when attempting to bind to the same address/port.
This patch sets NP_THREAD_ACTIVE before the np is placed on g_np_list,
so even if the thread hasn't run yet, iscsit_get_np will return the
existing np.
Also, convert np_lock -> np_mutex + hold across adding new net portal
to g_np_list to prevent a race where two threads may attempt to create
the same network portal, resulting in one of them failing.

In iSCSI negotiations with initiator CHAP enabled, usernames with
trailing garbage are permitted, because the string comparison only
checks the strlen of the configured username.
e.g. "usernameXXXXX" will be permitted to match "username".
Just check one more byte so the trailing null char is also matched.

extract_param() is called with max_length set to the total size of the
output buffer. It's not safe to allow a parameter length equal to the
buffer size as the terminating null would be written one byte past the
end of the output buffer.

This patch fixes spc_emulate_inquiry_std() to add trailing ASCII
spaces for INQUIRY vendor + model fields following SPC-4 text:
"ASCII data fields described as being left-aligned shall have any
unused bytes at the end of the field (i.e., highest offset) and
the unused bytes shall be filled with ASCII space characters (20h)."
This addresses a problem with Falconstor NSS multipathing.

If a key was larger than 64 bytes, as checked by iscsi_check_key(), the
error response packet, generated by iscsi_add_notunderstood_response(),
would still attempt to copy the entire key into the packet, overflowing
the structure on the heap.
Remote preauthentication kernel memory corruption was possible if a
target was configured and listening on the network.

Fix two issues in OOO commands processing done at iscsit_attach_ooo_cmdsn.
Handle command serial numbers wrap around by using iscsi_sna_lt and not regular comparisson.
The routine iterates until it finds an entry whose serial number is greater than the serial number of
the new one, thus the new entry should be inserted before that entry and not after.

This patch fixes a bug where a handful of informational / control CDBs
that should be allowed during ALUA access state Standby/Offline/Transition
where incorrectly returning CHECK_CONDITION + ASCQ_04H_ALUA_TG_PT_*.
This includes INQUIRY + REPORT_LUNS, which would end up preventing LUN
registration when LUN scanning occured during these ALUA access states.

The page++ is wrong. It makes bio_add_pc_page() pointing to a wrong page

This patch adds missing bounds checking for the configfs provided
mapped_lun value during target_fabric_make_mappedlun() setup ahead
of se_lun_acl initialization.
This addresses a potential OOPs when using a mapped_lun value that
exceeds the hardcoded TRANSPORT_MAX_LUNS_PER_TPG-1 value within
se_node_acl->device_list[].

This patch fixes a bug in core_tpg_check_initiator_node_acl() ->
core_tpg_get_initiator_node_acl() where a dynamically created
se_node_acl generated during session login would be skipped during
subsequent lookup due to the '!acl->dynamic_node_acl' check, causing
a new se_node_acl to be created with a duplicate ->initiatorname.
This would occur when a fabric endpoint was configured with
TFO->tpg_check_demo_mode()=1 + TPF->tpg_check_demo_mode_cache()=1
preventing the release of an existing se_node_acl during se_session
shutdown.
Also, drop the unnecessary usage of core_tpg_get_initiator_node_acl()
within core_dev_init_initiator_node_lun_acl() that originally
required the extra '!acl->dynamic_node_acl' check, and just pass
the configfs provided se_node_acl pointer instead.

This patch fixes a possible divide by zero bug when the fabric_max_sectors
device attribute is written and backend se_device failed to be successfully
configured -> enabled.
Go ahead and use block_size=512 within se_dev_set_fabric_max_sectors()
in the event of a target_configure_device() failure case, as no valid
dev->dev_attrib.block_size value will have been setup yet.

The UASP protocol does not inform the target device upfront how much
data it should expect so we have to learn in from the CDB. So in order
to handle this case, add a TARGET_SCF_UNKNOWN_SIZE to target_submit_cmd()
and perform an explictly assignment for se_cmd->data_length from the
extracted CDB size in transport_generic_cmd_sequencer().

This is to fix a regression that only affect the stable (not for the mainline)

This patch adds [dev,lun]_link_magic value assignment + checks within generic
target_fabric_port_link() and target_fabric_mappedlun_link() code to ensure
destination config_item *target_item sent from configfs_symlink() ->
config_item_operations->allow_link() is the underlying se_device->dev_group
and se_lun->lun_group that we expect to symlink.

When transport_lookup_tmr_lun() fails and we return a task management
response from target_complete_tmr_failure(), we need to call
transport_cmd_check_stop_to_fabric() to release the last ref to the
cmd after calling se_tfo->queue_tm_rsp(), or else we will never remove
the failed TMR from the session command list (and we'll end up waiting
forever when trying to tear down the session).

The lockdep warning below is in theory correct but it will be in really weird
rare situation that ends up that deadlock since the tcm fc session is hashed
based the rport id. Nonetheless, the complaining below is about rcu callback
that does the transport_deregister_session() is happening in softirq, where
transport_register_session() that happens earlier is not. This triggers the
lockdep warning below. So, just fix this to make lockdep happy by disabling
the soft irq before calling transport_register_session() in ft_prli.
BTW, this was found in FCoE VN2VN over two VMs, couple of create and destroy
would get this triggered.

The sleeping code in iscsi_target_tx_thread() is susceptible to the classic
missed wakeup race:
- TX thread finishes handle_immediate_queue() and handle_response_queue(),
thinks both queues are empty.
- Another thread adds a queue entry and does wake_up_process(), which does
nothing because the TX thread is still awake.
- TX thread does schedule_timeout() and sleeps forever.
In practice this can kill an iSCSI connection if for example an initiator
does single-threaded writes and the target misses the wakeup window when
queueing an R2T; in this case the connection will be stuck until the
initiator loses patience and does some task management operation (or kills
the connection entirely).
Fix this by converting to wait_event_interruptible(), which does not
suffer from this sort of race.

The expression (max_sectors * block_size) might overflow a u32
(indeed, since iblock sets max_hw_sectors to UINT_MAX, it is
guaranteed to overflow and end up with a much-too-small result in many
common cases). Fix this by doing an equivalent calculation that
doesn't require multiplication.
While we're touching this code, avoid splitting a printk format across
two lines and use pr_info(...) instead of printk(KERN_INFO ...).

If the call to core_dev_release_virtual_lun0() fails, then nothing
sets ret to anything other than 0, so even though everything is
torn down and freed, target_core_init_configfs() will seem to succeed
and the module will be loaded. Fix this by passing the return value
on up the chain.

Fabric drivers currently expect to internally release se_cmd in the event
of a TMR failure during target_submit_tmr(), which means the immediate call
to transport_generic_free_cmd() after TFO->queue_tm_rsp() from within
target_complete_tmr_failure() workqueue context is wrong.
This is done as some fabrics expect TMR operations to be acknowledged
before releasing the descriptor, so the assumption that core is releasing
se_cmd associated TMR memory is incorrect. This fixes a OOPs where
transport_generic_free_cmd() was being called more than once.
This bug was originally observed with tcm_qla2xxx fabric ports.
