Hugh Dickins 1448dc70cd mm: larger stack guard gap, between vmas

Stack guard page is a useful feature to reduce a risk of stack smashing
into a different mapping. We have been using a single page gap which
is sufficient to prevent having stack adjacent to a different mapping.
But this seems to be insufficient in the light of the stack usage in
userspace. E.g. glibc uses as large as 64kB alloca() in many commonly
used functions. Others use constructs liks gid_t buffer[NGROUPS_MAX]
which is 256kB or stack strings with MAX_ARG_STRLEN.

This will become especially dangerous for suid binaries and the default
no limit for the stack size limit because those applications can be
tricked to consume a large portion of the stack and a single glibc call
could jump over the guard page. These attacks are not theoretical,
unfortunatelly.

Make those attacks less probable by increasing the stack guard gap
to 1MB (on systems with 4k pages; but make it depend on the page size
because systems with larger base pages might cap stack allocations in
the PAGE_SIZE units) which should cover larger alloca() and VLA stack
allocations. It is obviously not a full fix because the problem is
somehow inherent, but it should reduce attack space a lot.

One could argue that the gap size should be configurable from userspace,
but that can be done later when somebody finds that the new 1MB is wrong
for some special case applications.  For now, add a kernel command line
option (stack_guard_gap) to specify the stack gap size (in page units).

Implementation wise, first delete all the old code for stack guard page:
because although we could get away with accounting one extra page in a
stack vma, accounting a larger gap can break userspace - case in point,
a program run with "ulimit -S -v 20000" failed when the 1MB gap was
counted for RLIMIT_AS; similar problems could come with RLIMIT_MLOCK
and strict non-overcommit mode.

Instead of keeping gap inside the stack vma, maintain the stack guard
gap as a gap between vmas: using vm_start_gap() in place of vm_start
(or vm_end_gap() in place of vm_end if VM_GROWSUP) in just those few
places which need to respect the gap - mainly arch_get_unmapped_area(),
and and the vma tree's subtree_gap support for that.

in-kernel dwarf unwinder has a fast binary lookup and a fallback linear
search (which iterates thru each of ~11K entries) thus takes 2 orders of
magnitude longer (~3 million cycles vs. 2000). Routines written in hand
assembler lack dwarf info (as we don't support assembler CFI pseudo-ops
yet) fail the unwinder binary lookup, hit linear search, failing
nevertheless in the end.

However the linear search is pointless as binary lookup tables are created
from it in first place. It is impossible to have binary lookup fail while
succeed the linear search. It is pure waste of cycles thus removed by
this patch.

This manifested as RCU stalls / NMI watchdog splat when running
hackbench under perf with callgraph profiling. The triggering condition
was perf counter overflowing in routine lacking dwarf info (like memset)
leading to patheic 3 million cycle unwinder slow path and by the time it
returned new interrupts were already pending (Timer, IPI) and taken
rightaway. The original memset didn't make forward progress, system kept
accruing more interrupts and more unwinder delayes in a vicious feedback
loop, ultimately triggering the NMI diagnostic.

Alexey Brodkin 508a750fe6 ARC: make sure instruction_pointer() returns unsigned value

Currently instruction_pointer() returns pt_regs->ret and so return value
is of type "long", which implicitly stands for "signed long".

While that's perfectly fine when dealing with 32-bit values if return
value of instruction_pointer() gets assigned to 64-bit variable sign
extension may happen.

And at least in one real use-case it happens already.
In perf_prepare_sample() return value of perf_instruction_pointer()
(which is an alias to instruction_pointer() in case of ARC) is assigned
to (struct perf_sample_data)->ip (which type is "u64").

And what we see if instuction pointer points to user-space application
that in case of ARC lays below 0x8000_0000 "ip" gets set properly with
leading 32 zeros. But if instruction pointer points to kernel address
space that starts from 0x8000_0000 then "ip" is set with 32 leadig
"f"-s. I.e. id instruction_pointer() returns 0x8100_0000, "ip" will be
assigned with 0xffff_ffff__8100_0000. Which is obviously wrong.

In particular that issuse broke output of perf, because perf was unable
to associate addresses like 0xffff_ffff__8100_0000 with anything from
/proc/kallsyms.

That's what we used to see:

A malicious signal handler / restorer can DOS the system by fudging the
user regs saved on stack, causing weird things such as sigreturn returning
to user mode PC but cpu state still being kernel mode....

Ensure that in sigreturn path status32 always has U bit; any other bogosity
(gargbage PC etc) will be taken care of by normal user mode exceptions mechanisms.

Reproducer signal handler:

The core VM already knows about VM_FAULT_SIGBUS, but cannot return a
"you should SIGSEGV" error, because the SIGSEGV case was generally
handled by the caller - usually the architecture fault handler.

That results in lots of duplication - all the architecture fault
handlers end up doing very similar "look up vma, check permissions, do
retries etc" - but it generally works.  However, there are cases where
the VM actually wants to SIGSEGV, and applications _expect_ SIGSEGV.

In particular, when accessing the stack guard page, libsigsegv expects a
SIGSEGV.  And it usually got one, because the stack growth is handled by
that duplicated architecture fault handler.

However, when the generic VM layer started propagating the error return
from the stack expansion in commit fee7e49d4514 ("mm: propagate error
from stack expansion even for guard page"), that now exposed the
existing VM_FAULT_SIGBUS result to user space.  And user space really
expected SIGSEGV, not SIGBUS.

To fix that case, we need to add a VM_FAULT_SIGSEGV, and teach all those
duplicate architecture fault handlers about it.  They all already have
the code to handle SIGSEGV, so it's about just tying that new return
value to the existing code, but it's all a bit annoying.

This is the mindless minimal patch to do this.  A more extensive patch
would be to try to gather up the mostly shared fault handling logic into
one generic helper routine, and long-term we really should do that
cleanup.

Just from this patch, you can generally see that most architectures just
copied (directly or indirectly) the old x86 way of doing things, but in
the meantime that original x86 model has been improved to hold the VM
semaphore for shorter times etc and to handle VM_FAULT_RETRY and other
"newer" things, so it would be a good idea to bring all those
improvements to the generic case and teach other architectures about
them too.

We used to calculate page address differently in 2 cases:

1. In virt_to_page(x) we do
 --->8---
 mem_map + (x - CONFIG_LINUX_LINK_BASE) >> PAGE_SHIFT
 --->8---

2. In in pte_page(x) we do
 --->8---
 mem_map + (pte_val(x) - PAGE_OFFSET) >> PAGE_SHIFT
 --->8---

That leads to problems in case PAGE_OFFSET != CONFIG_LINUX_LINK_BASE -
different pages will be selected depending on where and how we calculate
page address.

In particular in the STAR 9000853582 when gdb attempted to read memory
of another process it got improper page in get_user_pages() because this
is exactly one of the places where we search for a page by pte_page().

The fix is trivial - we need to calculate page address similarly in both
cases.

Johannes Weiner e2ec2c2b96 arch: mm: pass userspace fault flag to generic fault handler

Unlike global OOM handling, memory cgroup code will invoke the OOM killer
in any OOM situation because it has no way of telling faults occuring in
kernel context - which could be handled more gracefully - from
user-triggered faults.

Pass a flag that identifies faults originating in user space from the
architecture-specific fault handlers to generic code so that memcg OOM
handling can be improved.

Johannes Weiner 20c92c01bf arch: mm: remove obsolete init OOM protection

The memcg code can trap tasks in the context of the failing allocation
until an OOM situation is resolved.  They can hold all kinds of locks
(fs, mm) at this point, which makes it prone to deadlocking.

This series converts memcg OOM handling into a two step process that is
started in the charge context, but any waiting is done after the fault
stack is fully unwound.

Patches 1-4 prepare architecture handlers to support the new memcg
requirements, but in doing so they also remove old cruft and unify
out-of-memory behavior across architectures.

Patch 5 disables the memcg OOM handling for syscalls, readahead, kernel
faults, because they can gracefully unwind the stack with -ENOMEM.  OOM
handling is restricted to user triggered faults that have no other
option.

Patch 6 reworks memcg's hierarchical OOM locking to make it a little
more obvious wth is going on in there: reduce locked regions, rename
locking functions, reorder and document.

Patch 7 implements the two-part OOM handling such that tasks are never
trapped with the full charge stack in an OOM situation.

This patch:

Back before smart OOM killing, when faulting tasks were killed directly on
allocation failures, the arch-specific fault handlers needed special
protection for the init process.

Now that all fault handlers call into the generic OOM killer (see commit
609838cfed97: "mm: invoke oom-killer from remaining unconverted page
fault handlers"), which already provides init protection, the
arch-specific leftovers can be removed.

Johannes Weiner b13a714fb4 mm: invoke oom-killer from remaining unconverted page fault handlers

A few remaining architectures directly kill the page faulting task in an
out of memory situation.  This is usually not a good idea since that
task might not even use a significant amount of memory and so may not be
the optimal victim to resolve the situation.

Since 2.6.29's 1c0fe6e ("mm: invoke oom-killer from page fault") there
is a hook that architecture page fault handlers are supposed to call to
invoke the OOM killer and let it pick the right task to kill.  Convert
the remaining architectures over to this hook.

To have the previous behavior of simply taking out the faulting task the
vm.oom_kill_allocating_task sysctl can be set to 1.

Uwe Kleine-König 2b4cc3cb58 Kconfig: rename HAS_IOPORT to HAS_IOPORT_MAP
If the renamed symbol is defined lib/iomap.c implements ioport_map and
ioport_unmap and currently (nearly) all platforms define the port
accessor functions outb/inb and friend unconditionally.  So
HAS_IOPORT_MAP is the better name for this.

Consequently NO_IOPORT is renamed to NO_IOPORT_MAP.

The motivation for this change is to reintroduce a symbol HAS_IOPORT
that signals if outb/int et al are available.  I will address that at
least one merge window later though to keep surprises to a minimum and
catch new introductions of (HAS|NO)_IOPORT.

The changes in this commit were done using:

There was a very small race window where resume to kernel mode from a
Exception Path (or pure kernel mode which is true for most of ARC
exceptions anyways), was not disabling interrupts in restore_regs,
clobbering the exception regs

Anton found the culprit call flow (after many sleepless nights)

| 1. we got a Trap from user land
| 2. started to service it.
| 3. While doing some stuff on user-land memory (I think it is padzero()),
|     we got a DataTlbMiss
| 4. On return from it we are taking "resume_kernel_mode" path
| 5. NEED_RESHED is not set, so we go to "return from exception" path in
|     restore regs.
| 6. there seems to be IRQ happening

Vineet Gupta 27dd47db1b ARC: Entry Handler tweaks: Optimize away redundant IRQ_DISABLE_SAVE

In the exception return path, for both U/K cases, intr are already
disabled (for various existing reasons). So when we drop down to
@restore_regs, we need not redo that.

There was subtle issue - when intr were NOT being disabled for
ret-to-kernel-but-no-preemption case - now fixed by moving the
IRQ_DISABLE further up in @resume_kernel_mode.

So what do we gain:

* Shaves off a few insn in return path.

* Eliminates the need for IRQ_DISABLE_SAVE assembler macro for ARCv2
  hence allows for entry code sharing.

in the System C model. In 3.8, the 8250_dw driver didn't really use these

Johannes Weiner 0d49fb2dc7 arch: mm: pass userspace fault flag to generic fault handler
Unlike global OOM handling, memory cgroup code will invoke the OOM killer
in any OOM situation because it has no way of telling faults occuring in
kernel context - which could be handled more gracefully - from
user-triggered faults.

Pass a flag that identifies faults originating in user space from the
architecture-specific fault handlers to generic code so that memcg OOM
handling can be improved.

Most architectures use the same implementation. Collapse the common ones
into a single weak function that can be overridden.

Santosh Shilimkar 30909344af of: Specify initrd location using 64-bit
On some PAE architectures, the entire range of physical memory could reside
outside the 32-bit limit.  These systems need the ability to specify the
initrd location using 64-bit numbers.

This patch globally modifies the early_init_dt_setup_initrd_arch() function to
use 64-bit numbers instead of the current unsigned long.

There has been quite a bit of debate about whether to use u64 or phys_addr_t.
It was concluded to stick to u64 to be consistent with rest of the device
tree code. As summarized by Geert, "The address to load the initrd is decided
by the bootloader/user and set at that point later in time. The dtb should not
be tied to the kernel you are booting"

Jiang Liu b78e83204c mm: concentrate modification of totalram_pages into the mm core
Concentrate code to modify totalram_pages into the mm core, so the arch
memory initialized code doesn't need to take care of it.  With these
changes applied, only following functions from mm core modify global
variable totalram_pages: free_bootmem_late(), free_all_bootmem(),
free_all_bootmem_node(), adjust_managed_page_count().

With this patch applied, it will be much more easier for us to keep
totalram_pages and zone->managed_pages in consistence.

Vineet Gupta 40a894023d ARC: Incorrect mm reference used in vmalloc fault handler
commit 9c41f4eeb9d51f3ece20428d35a3ea32cf3b5622 upstream.

A vmalloc fault needs to sync up PGD/PTE entry from init_mm to current
task's "active_mm".  ARC vmalloc fault handler however was using mm.

A vmalloc fault for non user task context (actually pre-userland, from
init thread's open for /dev/console) caused the handler to deref NULL mm
(for mm->pgd)

The reasons it worked so far is amazing:

1. By default (!SMP), vmalloc fault handler uses a cached value of PGD.
   In SMP that MMU register is repurposed hence need for mm pointer deref.

2. In pre-3.12 SMP kernel, the problem triggering vmalloc didn't exist in
   pre-userland code path - it was introduced with commit 20bafb3d23d108bc
   "n_tty: Move buffers into n_tty_data"

Vineet Gupta 4b3ea63f5a ARC: Ignore ptrace SETREGSET request for synthetic register "stop_pc"

ARCompact TRAP_S insn used for breakpoints, commits before exception is
taken (updating architectural PC). So ptregs->ret contains next-PC and
not the breakpoint PC itself. This is different from other restartable
exceptions such as TLB Miss where ptregs->ret has exact faulting PC.
gdb needs to know exact-PC hence ARC ptrace GETREGSET provides for
@stop_pc which returns ptregs->ret vs. EFA depending on the
situation.

However, writing stop_pc (SETREGSET request), which updates ptregs->ret
doesn't makes sense stop_pc doesn't always correspond to that reg as
described above.

This was not an issue so far since user_regs->ret / user_regs->stop_pc
had same value and both writing to ptregs->ret was OK, needless, but NOT
broken, hence not observed.

With gdb "jump", they diverge, and user_regs->ret updating ptregs is
overwritten immediately with stop_pc, which this patch fixes.

Christian Ruppert 19a420033d ARC: Fix signal frame management for SA_SIGINFO

Previously, when a signal was registered with SA_SIGINFO, parameters 2
and 3 of the signal handler were written to registers r1 and r2 before
the register set was saved. This led to corruption of these two
registers after returning from the signal handler (the wrong values were
restored).
With this patch, registers are now saved before any parameters are
passed, thus maintaining the processor state from before signal entry.

Vineet Gupta 0c06a0a693 ARC: Workaround spinlock livelock in SMP SystemC simulation
commit 6c00350b573c0bd3635436e43e8696951dd6e1b6 upstream.

Some ARC SMP systems lack native atomic R-M-W (LLOCK/SCOND) insns and
can only use atomic EX insn (reg with mem) to build higher level R-M-W
primitives. This includes a SystemC based SMP simulation model.

So rwlocks need to use a protecting spinlock for atomic cmp-n-exchange
operation to update reader(s)/writer count.

The spinlock operation itself looks as follows:

	mov reg, 1		; 1=locked, 0=unlocked
retry:
	EX reg, [lock]		; load existing, store 1, atomically
	BREQ reg, 1, rety	; if already locked, retry

In single-threaded simulation, SystemC alternates between the 2 cores
with "N" insn each based scheduling. Additionally for insn with global
side effect, such as EX writing to shared mem, a core switch is
enforced too.

Given that, 2 cores doing a repeated EX on same location, Linux often
got into a livelock e.g. when both cores were fiddling with tasklist
lock (gdbserver / hackbench) for read/write respectively as the
sequence diagram below shows:

The fix was to unlock using the EX insn too (step 7), to trigger another
SystemC scheduling pass which would let core1 proceed, eliding the
livelock.

 | LTP tests syscalls/process_vm_readv01 and process_vm_writev01 fail
 | similarly in one testcase test_iov_invalid -> lvec->iov_base.
 | Testcase expects errno EFAULT and return code -1,
 | but it gets return code 1 and ERRNO is 0 what means success.

Essentially test case was passing a pointer of -1 which access_ok()
was not catching. It was doing [@addr + @sz <= TASK_SIZE] which would
pass for @addr == -1

Fixed that by rewriting as [@addr <= TASK_SIZE - @sz]

Cast usecs to u64, to ensure that the (usecs * 4295 * HZ)
multiplication is 64 bit.

Initially, the (usecs * 4295 * HZ) part was done as a 32 bit
multiplication, with the result casted to 64 bit. This led to some bits
falling off, causing a "DMA initialization error" in the stmmac Ethernet
driver, due to a premature timeout.

Noam Camus 98f745546b ARC: SMP failed to boot due to missing IVT setup

Commit 05b016ecf5e7a "ARC: Setup Vector Table Base in early boot" moved
the Interrupt vector Table setup out of arc_init_IRQ() which is called
for all CPUs, to entry point of boot cpu only, breaking booting of others.

Fix by adding the same to entry point of non-boot CPUs too.

read_arc_build_cfg_regs() printing IVT Base Register didn't help the
casue since it prints a synthetic value if zero which is totally bogus,
so fix that to print the exact Register.

Vineet Gupta 9b2c750d8e ARC: gdbserver breakage in Big-Endian configuration #1
[Based on mainline commit 502a0c775c7f0a: "ARC: pt_regs update #5"]

gdbserver needs @stop_pc, served by ptrace, but fetched from pt_regs
differently, based on in_brkpt_traps(), which in turn relies on
additional machine state in pt_regs->event bitfield.

        unsigned long orig_r8:16, event:16;

For big endian config, this macro was returning false, despite being in
breakpoint Trap exception, causing wrong @stop_pc to be returned to gdb.

Issue #1: In BE, @event above is at offset 2 in word, while a STW insn
          at offset 0 was used to update it. Resort to using ST insn
	  which updates the half-word at right location.

Issue #2: The union involving bitfields causes all the members to be
	  laid out at offset 0. So with fix #1 above, ASM was now
	  updating at offset 2, "C" code was still referencing at
	  offset 0. Fixed by wrapping bitfield in a struct.
