When reading an extra descriptor, we need to properly check the minimum
and maximum size allowed, to prevent from invalid data being sent by a
device.

Andrey Konovalov reported a possible out-of-bounds problem for the
cdc_parse_cdc_header function. He writes:
It looks like cdc_parse_cdc_header() doesn't validate buflen
before accessing buffer[1], buffer[2] and so on. The only check
present is while (buflen > 0).
So fix this issue up by properly validating the buffer length matches
what the descriptor says it is.

Parsing CDC headers a buffer overflow cannot just be prevented
by checking that the remainder of the buffer is longer than minimum
length. The size of the fields to be parsed must be figured in, too.
In newer kernels this issue has been fixed at a central location with
commit 2e1c42391ff2556387b3cb6308b24f6f65619feb

Phil and I found out a problem with commit:
7e860a6e7aa6 ("cdc-acm: add sanity checks")
It added some sanity checks to ignore potential garbage in CDC headers but
also introduced a potential infinite loop. This can happen at the first
loop iteration (elength = 0 in that case) if the description isn't a
DT_CS_INTERFACE or later if 'buffer[0]' is zero.
It should also be noted that the wrong length was being added to 'buffer'
in case 'buffer[1]' was not a DT_CS_INTERFACE descriptor, since elength was
assigned after that check in the loop.
A specially crafted USB device could be used to trigger this infinite loop.

If acm_submit_read_urbs() fails in acm_port_activate(), error handling
code calls usb_autopm_put_interface() while it is already called
before acm_submit_read_urbs(). The patch reorganizes error handling code
to avoid double decrement of USB interface's PM-usage counter.
Found by Linux Driver Verification project (linuxtesting.org).

USBTMC devices are required to have a bulk-in and a bulk-out endpoint,
but the driver failed to verify this, something which could lead to the
endpoint addresses being taken from uninitialised memory.
Make sure to zero all private data as part of allocation, and add the
missing endpoint sanity check.
Note that this also addresses a more recently introduced issue, where
the interrupt-in-presence flag would also be uninitialised whenever the
optional interrupt-in endpoint is not present. This in turn could lead
to an interrupt urb being allocated, initialised and submitted based on
uninitialised values.

Some modems, such as the Telit UE910, are using an Infineon Flash Loader
utility. It has two interfaces, 2/2/0 (Abstract Modem) and 10/0/0 (CDC
Data). The latter can be used as a serial interface to upgrade the
firmware of the modem. However, that isn't possible when the cdc-acm
driver takes control of the device.
The following is an explanation of the behaviour by Daniele Palmas during
discussion on linux-usb.

It is not permitted to set task state before lock. usblp_wwait sets
the state to TASK_INTERRUPTIBLE and calls mutex_lock_interruptible.
Upon return from that function, the state will be TASK_RUNNING again.

Make sure to only raise DTR on transitions from B0 in set_termios.
Also allow set_termios to be called from open with a termios_old of
NULL. Note that DTR will not be raised prematurely in this case.

In addition to bringing in upstream commits, this merge also makes minor
changes to mainitain compatibility with upstream:
The definition of list_next_entry in qcrypto.c and ipa_dp.c has been
removed, as upstream has moved the definition to list.h. The implementation
of list_next_entry was identical between the two.
irq.c, for both arm and arm64 architecture, has had its calls to
__irq_set_affinity_locked updated to reflect changes to the API upstream.
Finally, as we have removed the sleep_length member variable of the
tick_sched struct, all changes made by upstream commit ec804bd do not
apply to our tree and have been removed from this merge. Only
kernel/time/tick-sched.c is impacted.

Make sure only to decrement the PM counters if they were actually
incremented.
Note that the USB PM counter, but not necessarily the driver core PM
counter, is reset when the interface is unbound.

We should stop I/O unconditionally at suspend rather than rely on the
tty-port initialised flag (which is set prior to stopping I/O during
shutdown) in order to prevent suspend returning with URBs still active.

The current ACM runtime-suspend implementation is broken in several
ways:
Firstly, it buffers only the first write request being made while
suspended -- any further writes are silently dropped.
Secondly, writes being dropped also leak write urbs, which are never
reclaimed (until the device is unbound).
Thirdly, even the single buffered write is not cleared at shutdown
(which may happen before the device is resumed), something which can
lead to another urb leak as well as a PM usage-counter leak.
Fix this by implementing a delayed-write queue using urb anchors and
making sure to discard the queue properly at shutdown.

Fix race between write() and resume() due to improper locking that could
lead to writes being reordered.
Resume must be done atomically and susp_count be protected by the
write_lock in order to prevent racing with write(). This could otherwise
lead to writes being reordered if write() grabs the write_lock after
susp_count is decremented, but before the delayed urb is submitted.

Fix race between write() and suspend() which could lead to writes being
dropped (or I/O while suspended) if the device is runtime suspended
while a write request is being processed.
Specifically, suspend() releases the write_lock after determining the
device is idle but before incrementing the susp_count, thus leaving a
window where a concurrent write() can submit an urb.

By specifying NO_UNION_NORMAL the ACM driver does only use the first two

The bulk message timeout is 1 second by default. Some cards take
more time to respond to CCID commands. Provide a module param to
configure the bulk message timeout.

The URBs are killed in disconnect method. If the device is disconnected
before the /dev/ccid_bridge is closed, no need to kill the URBs again.
As the URB reference count is dropped in disconnect, accessing it in
release is causing the kernel panic.

usb device ref count is incremented in probe via usb_get_dev. Call
usb_put_dev during disconnect to drop the ref count. Otherwise
struct usb_device memory is leaked.

The card is taking more than 500 msec to respond to some commands.
Increase the timeout to 1 sec.

This module implements a bridge driver which allows user space
to communicate with USB smart card devices. This module provides
/dev/ccid_bridge device file which allows read/write/ioctl methods.
The ioctls facilitate control and interrupt transfers. The bulk
transfers are implemented via read and write. The interrupt
endpoint is optional and not all devices may support it.

Inside usbtmc_ioctl_clear_out_halt()/usbtmc_ioctl_clear_in_halt(),
usb_clear_halt() needn't any buffer to pass in, so remove the
unnecessary memory allocation.

This patch removes the depends on USB from all config symbols in
drivers/usb/host/Kconfig and replace that with an if USB / endif block
as suggested by Alan Stern. Some source ... Kconfig lines have been
shuffled around to permit a better regroupment of the Kconfig files
depending on "config USB" item. No functionnal change is introduced.
