if built statically enforce exclusivity

- saa7134-oss is now a standalone module as well
- remaining DMA sound code has been removed from core the module
- Lots of small cleanups and variable renames to get more consistency
between the OSS and ALSA drivers
- Fixed saa7134-alsa spinlock bug
- Added missing #include in saa7134-oss

The #define CONFIG_DVB_* are actually CFLAGS set by Makefile. CONFIG_*
namespace is reserved for Kconfig. This renames them back to HAVE_*

This patch moves #define from cx88-dvb.c and saa7134-dvb.c into Makefile as
CFLAGS, allowing code compatability with video4linux cvs.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
