Commit 8520f38099 ("USB: change hub initialization sleeps to
delayed_work") changed the hub_activate() routine to make part of it
run in a workqueue. However, the commit failed to take a reference to
the usb_hub structure or to lock the hub interface while doing so. As
a result, if a hub is plugged in and quickly unplugged before the work
routine can run, the routine will try to access memory that has been
deallocated. Or, if the hub is unplugged while the routine is
running, the memory may be deallocated while it is in active use.
This patch fixes the problem by taking a reference to the usb_hub at
the start of hub_activate() and releasing it at the end (when the work
is finished), and by locking the hub interface while the work routine
is running. It also adds a check at the start of the routine to see
if the hub has already been disconnected, in which nothing should be
done.

Some devices I got show an inability to operate right after
power on if they are already connected. They are beyond recovery
if the descriptors are requested multiple times. So in case of
a timeout we rather bail early and reset again. But it must be
done only on the first loop lest we get into a reset/time out
spiral that can be overcome with a retry.
This patch is a rework of a patch that fell through the cracks.

Some USB device / host controller combinations seem to have problems
with Link Power Management. For example, Steinar found that his xHCI
controller wouldn't handle bandwidth calculations correctly for two
video cards simultaneously when LPM was enabled, even though the bus
had plenty of bandwidth available.
This patch introduces a new quirk flag for devices that should remain
disabled for LPM, and creates quirk entries for Steinar's devices.

This full-speed USB device generates spurious remote wakeup event
as soon as USB_DEVICE_REMOTE_WAKEUP feature is set. As the result,
Linux can't enter system suspend and S0ix power saving modes once
this keyboard is used.
This patch tries to introduce USB_QUIRK_IGNORE_REMOTE_WAKEUP quirk.
With this quirk set, wakeup capability will be ignored during
device configure.
This patch could be back-ported to kernels as old as 2.6.39.

When we are doing compliance test with xHCI, we found that if we
enable CONFIG_USB_SUSPEND and plug in a bad device which causes
over-current condition to the root port, software will not be noticed.
The reason is that current code don't set hub->change_bits in
hub_activate() when over-current happens, and then hub_events() will
not check the port status because it thinks nothing changed.
If CONFIG_USB_SUSPEND is disabled, the interrupt pipe of the hub will
report the change and set hub->event_bits, and then hub_events() will
check what events happened.In this case over-current can be detected.

During surprise device hotplug removal tests, it was observed that
hub_events may try to call usb_lock_device on a device that has already
been freed. Protect the usb_device by taking out a reference (under the
hub_event_lock) when hub_events pulls it off the list, returning the
reference after hub_events is finished using it.

If user specifies that USB autosuspend must be disabled by module
parameter "usbcore.autosuspend=-1" then we must prevent
autosuspend of USB hub devices as well.
commit 596d789a21 introduced in v3.8 changed the original behaivour
and stopped respecting the usbcore.autosuspend parameter for hubs.

Problem Summary: Problem has been observed generally with PM states
where VBUS goes off during suspend. There are some SS USB devices which
take longer time for link training compared to many others. Such
devices fail to reconnect with same old address which was associated
with it before suspend.
When system resumes, at some point of time (dpm_run_callback->
usb_dev_resume->usb_resume->usb_resume_both->usb_resume_device->
usb_port_resume) SW reads hub status. If device is present,
then it finishes port resume and re-enumerates device with same
address. If device is not present then, SW thinks that device was
removed during suspend and therefore does logical disconnection
and removes all the resource allocated for this device.
Now, if I put sufficient delay just before root hub status read in
usb_resume_device then, SW sees always that device is present. In normal
course(without any delay) SW sees that no device is present and then SW
removes all resource associated with the device at this port. In the
latter case, after sometime, device says that hey I am here, now host
enumerates it, but with new address.
Problem had been reproduced when I connect verbatim USB3.0 hard disc
with my STiH407 XHCI host running with 3.10 kernel.
I see that similar problem has been reported here.

Reading above it seems that bug was not in 3.6.6 and was present in 3.8
and again it was not present for some in 3.12.6, while it was present
for few others. I tested with 3.13-FC19 running at i686 desktop, problem
was still there. However, I was failed to reproduce it with 3.16-RC4
running at same i686 machine. I would say it is just a random
observation. Problem for few devices is always there, as I am unable to
find a proper fix for the issue.
So, now question is what should be the amount of delay so that host is
always able to recognize suspended device after resume.
XHCI specs 4.19.4 says that when Link training is successful, port sets
CSC bit to 1. So if SW reads port status before successful link
training, then it will not find device to be present. USB Analyzer log
with such buggy devices show that in some cases device switch on the
RX termination after long delay of host enabling the VBUS. In few other
cases it has been seen that device fails to negotiate link training in
first attempt. It has been reported till now that few devices take as
long as 2000 ms to train the link after host enabling its VBUS and
RX termination. This patch implements a 2000 ms timeout for CSC bit to set
ie for link training. If in a case link trains before timeout, loop will
exit earlier.
This patch implements above delay, but only for SS device and when
persist is enabled.
So, for the good device overhead is almost none. While for the bad
devices penalty could be the time which it take for link training.
But, If a device was connected before suspend, and was removed
while system was asleep, then the penalty would be the timeout ie
2000 ms.
Results:
Verbatim USB SS hard disk connected with STiH407 USB host running 3.10
Kernel resumes in 461 msecs without this patch, but hard disk is
assigned a new device address. Same system resumes in 790 msecs with
this patch, but with old device address.

In addition to bringing in upstream commits, this merge also makes minor
changes to mainitain compatibility with upstream:
The definition of list_next_entry in qcrypto.c and ipa_dp.c has been
removed, as upstream has moved the definition to list.h. The implementation
of list_next_entry was identical between the two.
irq.c, for both arm and arm64 architecture, has had its calls to
__irq_set_affinity_locked updated to reflect changes to the API upstream.
Finally, as we have removed the sleep_length member variable of the
tick_sched struct, all changes made by upstream commit ec804bd do not
apply to our tree and have been removed from this merge. Only
kernel/time/tick-sched.c is impacted.

When using USB 3.0 pen drive with the [AMD] FCH USB XHCI Controller
[1022:7814], the second hotplugging will experience the USB 3.0 pen
drive is recognized as high-speed device. After bisecting the kernel,
I found the commit number 41e7e056cd
(USB: Allow USB 3.0 ports to be disabled.) causes the bug. After doing
some experiments, the bug can be fixed by avoiding executing the function
hub_usb3_port_disable(). Because the port status with [AMD] FCH USB
XHCI Controlleris [1022:7814] is already in RxDetect
(I tried printing out the port status before setting to Disabled state),
it's reasonable to check the port status before really executing
hub_usb3_port_disable().

Not all host controller drivers have bus-suspend and bus-resume
methods. When one doesn't, it will cause problems if runtime PM is
enabled in the kernel. The PM core will attempt to suspend the
controller's root hub, the suspend will fail because there is no
bus-suspend routine, and a -EBUSY error code will be returned to the
PM core. This will cause the suspend attempt to be repeated shortly
thereafter, in a never-ending loop.
Part of the problem is that the original error code -ENOENT gets
changed to -EBUSY in usb_runtime_suspend(), on the grounds that the PM
core will interpret -ENOENT as meaning that the root hub has gotten
into a runtime-PM error state. While this change is appropriate for
real USB devices, it's not such a good idea for a root hub. In fact,
considering the root hub to be in a runtime-PM error state would not
be far from the truth. Therefore this patch updates
usb_runtime_suspend() so that it adjusts error codes only for
non-root-hub devices.
Furthermore, the patch attempts to prevent the problem from occurring
in the first place by not enabling runtime PM by default for root hubs
whose host controller driver doesn't have bus_suspend and bus_resume
methods.

There is a possibility of root hub usb device being
freed as part of controller driver unbind and hub
thread trying to access root hub usb device. Hence
increment the usb device reference count before
accessing it by hub thread.

When a driver doesn't have pre_reset, post_reset, or reset_resume
methods, the USB core unbinds that driver when its device undergoes a
reset or a reset-resume, and then rebinds it afterward.
The existing straightforward implementation can lead to problems,
because each interface gets unbound and rebound before the next
interface is handled. If a driver claims additional interfaces, the
claim may fail because the old binding instance may still own the
additional interface when the new instance tries to claim it.
This patch fixes the problem by first unbinding all the interfaces
that are marked (i.e., their needs_binding flag is set) and then
rebinding all of them.
The patch also makes the helper functions in driver.c a little more
uniform and adjusts some out-of-date comments.

Commit 9df89d85b407690afa46ddfbccc80bec6869971d "usbcore: set
lpm_capable field for LPM capable root hubs" was created under the
assumption that all USB host controllers should have USB 3.0 Link PM
enabled for all devices under the hosts.
Unfortunately, that's not the case. The xHCI driver relies on knowledge
of the host hardware scheduler to calculate the LPM U1/U2 timeout
values, and it only sets lpm_capable to one for Intel host controllers
(that have the XHCI_LPM_SUPPORT quirk set).
When LPM is enabled for some Fresco Logic hosts, it causes failures with
a AgeStar 3UBT USB 3.0 hard drive dock:

There is no need to skip querying the config and string descriptors for
unauthorized WUSB devices when usb_new_device is called. It is allowed
by WUSB spec. The only action that needs to be delayed until
authorization time is the set config. This change allows user mode
tools to see the config and string descriptors earlier in enumeration
which is needed for some WUSB devices to function properly on Android
systems. It also reduces the amount of divergent code paths needed
for WUSB devices.

There is a race in the hub driver between hub_disconnect() and
recursively_mark_NOTATTACHED(). This race can be triggered if the
driver is unbound from a device at the same time as the bus's root hub

One problem is that recursively_mark_NOTATTACHED() uses the intfdata
value and hub->hdev->maxchild while hub_disconnect() is clearing them.
Another problem is that it uses hub->ports[i] while the port device is
being released.
To fix this race, we need to hold the device_state_lock while
hub_disconnect() changes the values. (Note that usb_disconnect()
and hub_port_connect_change() already acquire this lock at similar
critical times during a USB device's life cycle.) We also need to
remove the port devices after maxchild has been set to 0, instead of
before.

HCD_OLD_ENUM HCD flag is introduced to use old (or short) enumeration
scheme for the devices attached on the corresponding USB bus. The old
enumeration scheme is used by the MSM HSIC controller to avoid multiple
reset during enumeration. This is useful when a known device is attached
to the USB bus, which is the case with MSM HSIC. When a HSIC HUB is
connected, any device can be connected on the HSIC USB bus. The new
enumeration scheme supports wide variety of USB devices. Hence limit
the old enumeration scheme request of a HCD to only root ports.

This patch adds a check for USB_STATE_NOTATTACHED to the
hub_port_warm_reset_required() workaround for ports that end up in
Compliance Mode in hub_events() when trying to decide which reset
function to use. Trying to call usb_reset_device() with a NOTATTACHED
device will just fail and leave the port broken.

This patch adds the Port Reset Change flag to the set of bits that are
preemptively cleared on init/resume of a hub. In theory this bit should
never be set unexpectedly... in practice it can still happen if BIOS,
SMM or ACPI code plays around with USB devices without cleaning up
correctly. This is especially dangerous for XHCI root hubs, which don't
generate any more Port Status Change Events until all change bits are
cleared, so this is a good precaution to have (similar to how it's
already done for the Warm Port Reset Change flag).

If the hub_configure() fails after setting the hdev->maxchild
the hub->ports might be NULL or point to uninitialized kzallocated
memory causing NULL pointer dereference in hub_quiesce() during cleanup.
Now after such error the hdev->maxchild is set to 0 to avoid cleanup
of uninitialized ports.

Current driver is accessing the buffer memory after it is freed.
This could result in getting wrong value and might could cause
memory corruption. Hence fix this issue by taking idvendor value

The pm qos NO_POWER_OFF flag is checked twice during usb device suspend
to see if the usb port power off condition is met. This is redundant and
also will prevent the port from being powered off if the NO_POWER_OFF
flag is changed to 1 from 0 after the device was already suspended.

The hub driver's usb_port_suspend() routine doesn't handle errors
related to Link Power Management properly. It always returns failure,
it doesn't try to clean up the wakeup setting, (in the case of system
sleep) it doesn't try to go ahead with the port suspend regardless,
and it doesn't try to apply the new power-off mechanism.
This patch fixes these problems.
Note: Sarah fixed this patch to apply against 3.11, since the original
commit (4fae6f0fa86f92e6bc7429371b1e177ad0aaac66 "USB: handle LPM errors
during device suspend correctly") called usb_disable_remote_wakeup,
which won't be added until 3.12.
This patch should be backported to kernels as old as 3.5, that
contain the commit 8306095fd2 "USB:
Disable USB 3.0 LPM in critical sections.". There will be merge
conflicts, since LTM wasn't added until 3.6.

There could be race condition between h/w initiating sof
(start of frames) and finishing port reset. This can
cause hsic phy lockup, which results in enumeration failure.
Avoid this race condition by:
1. Moving to the old enumeration sequence where set_address is
initiated before get_device descriptor. This results in one
port reset instead of two port resets.
2. Halt usb controller before initiating port reset and start it after
port reset is complete. This is tricky because PORT_RESET bit
automatically clears when h/w completes the reset and failure to
start the controller within 3ms causes the peripheral device to
suspend resulting in enumeration failure. Hence, after setting
port reset bit forcefully drive strobe/data lines to reset using
ulpi interface and once port reset is complete, disable forceful
reset and start the controller in atomic context.

1. Add OTG PET device to TPL. OTG device shall support this
device for allowing compliance automated testing.
2. Add otg_srp_reqd filed to gadget. OTG B-device shall enable
this flag when OTG PET (Protocol and Electrical Tester) that
acts as A-device sends Set Feature TEST_MODE with wIndex high
byte value = 0x06. OTG PET expects B-device to initiate SRP
after the end of current session.
3. Add otg_vbus_off to usb_bus. USB core enables this flag
when OTG PET enumerates with bcdDevice[0] field in its Device
Descriptor is equal to 1. OTG PET expects A-device to turn off
the VBUS with in 5 sec of its disconnection which allows it to
initiate SRP.
3. Add support to identify OTG PET and start HNP quickly.

During enumeration of a device, USB core first resets parent
HUB's port then issues a GetDescriptor request. After this USB
core again issues a RESET followed by SetAddress and
GetDescriptor requests.
Some HSET Test devices, needed for USB-IF host compliance tests,
fail to enumerate due to change in speed after second reset.
Hence, avoid issuing a second RESET while dealing with a HSET
Test device.

hnp_supported flag is set for a USB bus when a HNP supported device
is connected on OTG port. This flag is used to schedule and cancel
HNP polling work. Clear this flag upon device disconnect. Otherwise
HNP polling work is scheduled when another device is connected on OTG
port though the device is not a OTG device.

OTG specification mandates an explicit message like "HUB device
not supported" to be displayed to user if host does not support
HUB class peripherals. Distinguish HUB class peripheral from
other unsupported peripherals and send the corresponding event.

Host controller driver may register root hub during HNP while
becoming B-Host. Remove delays involved in debouncing and
applying port power. This helps B-device to meet TB_ACON_BSE0
timing parameter and A-device will not find bus idle for longer
than TA_BIDL_ADIS.

ADP (Attach Detection Protocol) is introduced to detect remote
device presence with out applying power on VBUS. A-device
performs ADP probing and applies power on VBUS when remote device
is attached. B-device perform ADP sensing and request session
via SRP when remote device is attached. B-device can claim their
ADP capability in OTG descriptor.
HNP polling is introduced for peripheral device to notify its wish
to become host. Host device polls (i.e execute GetStatus())
peripheral and suspend the bus when peripheral returns host_request
True.
a_alt_hnp_support feature is obsoleted and a_hnp_support feature is
limited to legacy B-devices i.e compliant to older than Rev 2.0 spec.

Internal HUB present in ISP1763A sometimes returns 2 bytes of
data in response to GetPortStatus request. This is observed during
mutiple iterations of suspend/resume with ISP1763A.
As part of workaround retry GetPortStatus request if HUB
returns 2 bytes.

A user can initiate host compliance tests or suspend/resume
requests over the ISP1763A USB Host controller by issuing
IOCTLs on the pehci device.
Add necessary changes in USB core to support these requests.
These changes are taken from the 1.0.6.0 version of ST-E pehci
driver present at: http://www.box.net/shared/static/o5esh7r14q.zip
along with the fixes for checkpatch errors and removal of unused
variables.

The USB hub driver's event handler contains a check to catch SuperSpeed
devices that transitioned into the SS.Inactive state and tries to fix
them with a reset. It decides whether to do a plain hub port reset or
call the usb_reset_device() function based on whether there was a device
attached to the port.
However, there are device/hub combinations (found with a JetFlash
Transcend mass storage stick (8564:1000) on the root hub of an Intel
LynxPoint PCH) which can transition to the SS.Inactive state on
disconnect (and stay there long enough for the host to notice). In this
case, above-mentioned reset check will call usb_reset_device() on the
stale device data structure. The kernel will send pointless LPM control
messages to the no longer connected device address and can even cause
several 5 second khubd stalls on some (buggy?) host controllers, before
finally accepting the device's fate amongst a flurry of error messages.
This patch makes the choice of reset dependent on the port status that
has just been read from the hub in addition to the existence of an
in-kernel data structure for the device, and only proceeds with the more
extensive reset if both are valid.

The hub driver was recently changed to use "global" suspend for system
suspend transitions on non-SuperSpeed buses. This means that we don't
suspend devices individually by setting the suspend feature on the
upstream hub port; instead devices all go into suspend automatically
when the root hub stops transmitting packets. The idea was to save
time and to avoid certain kinds of wakeup races.
Now it turns out that many hubs are buggy; they don't relay wakeup
requests from a downstream port to their upstream port if the
downstream port's suspend feature is not set (depending on the speed
of the downstream port, whether or not the hub is enabled for remote
wakeup, and possibly other factors).
We can't have hubs dropping wakeup requests. Therefore this patch
goes partway back to the old policy: It sets the suspend feature for a
port if the device attached to that port or any of its descendants is
enabled for wakeup. People will still be able to benefit from the
time savings if they don't care about wakeup and leave it disabled on
all their devices.
In order to accomplish this, the patch adds a new field to the usb_hub
structure: wakeup_enabled_descendants is a count of how many devices
below a suspended hub are enabled for remote wakeup. A corresponding
new subroutine determines the number of wakeup-enabled devices at or
below an arbitrary suspended USB device.
This should be applied to the 3.10 stable kernel.

Control transfers have both IN and OUT (or SETUP) packets, so when
clearing TT buffers for a control transfer it's necessary to send
two HUB_CLEAR_TT_BUFFER requests to the hub.
